How to File your DOST Final Clearance in Cebu

If you haven't known, I took up Engineering way back 2003 to take up ECE (Electronics Engineering).
Yeah, I know this is a bit off from this beauty blog. *wink* I hope this post will help you somehow.

I was not really interested to take up that course since It never came to my mind ever since to become an engineer. This all happened when I passed the scholarship examination for DOST (Dept of Science and Technology) here in Cebu, Philippines.

I was glad, I passed the exam! And after five years.... Bam!

You know, I have always wanted to take up medical course or something like Psychology.
Anyways, If you reached the full term of service and would want to file for your FINAL CLEARANCE, below are the requirements:

• Photocopy of TOR (Transcript of Records)
• Photocopy of Diploma
• Photocopy of Certificate/s of Employment (equivalent to the number of years you enjoyed the scholarship)
• Letter of Intent (addressed to:
• Elizabeth Fontanilla - Director, ALS - DOST and Officer-in-Charge, SEI

• DOST Tracer Form (available at DOST Region 7 Office, Sudlon Lahug Cebu)

Submit all your documents to the office, and the staff will do the filing for you. Since the documents will be submitted to the Main Office in Manila. Wait for two weeks and your most awaited final clearance should be crisp and ready.

It was out of the blue, unplanned getaway that we had last December of 2011. I had the perfect time to visit a resort finally in Danao. It is a well managed resort I must say plus the place is really clean and the rooms are great as well. Cable TV is available except that at that time we went there WIFI was not really a need since smartphones were in-existent for us then.

We arrived there at around 10 in the evening and then went about our usual preparation and booked ahead obviously since it was peak season.

 LOCATION

Elsalvador Beach Resort is located in the heart of the Philippines- the central visayas region. It sits on the eastern coast of mid-north Cebu, 33 kilometers north of Cebu City, particularly situated in barangay Sabang, Danao City, Cebu province, Island of Cebu. The resort enjoys the cool breeze and the charming sunrise of the east. The resort is very accessible being located along the northern coastal road of Cebu, 50 minutes of sight seeing ride from the Cebu international Port and 45 minutes from Mactan-Cebu International Airport. We enjoyed our stay plus the staff is very accommodating. We loved the food as well.

Secured at Local Civil Registrar (LCR) office located at the Ground Floor City Health Building, General Maxilom Extension, Cebu City, Phils. | Contact Nos. (6332) 233-2484 & 232-6664

• Certificate of Attendance in Pre-Marital Counseling and Family Planning Seminar Issued by the HRD-Office (Ask for Schedules) √
  • 1 original copy and 1 photocopy (Seminar: Room 104)
• Birth Certificate (New) √
  • Issued by NSO (National Statistics Office) 1 original copy and 2 photocopies
• Certificate of No Marriage (CENOMAR)
  • Issued by NSO (National Statistics Office)
  • 1 original copy and 1 photocopy
• Cedula with signature (Community Tax No. For the Current Year)
  • 1 original copy and 2 photocopies
• Barangay Certificate √
  • 1 original copy and 1 photocopy
• For ages 18-24: Appearance of both parents, with valid I.D., for parental consent/advice
• Valid ID, 1 original and 2 photocopies √

There is a 10 Day Waiting Period from filing of application.  The Marriage License is valid for 120 days (4months)

Regarding NSO Requests for Certifications: The interested party shall provide the following information to facilitate verification and issuance of certification.
Birth Certificate  (will take 3 days to process)

1. Complete name of the child (first, middle, last)
2. Complete name of the father
3. Complete maiden name of the mother
4. Date of birth (month, day, year)
5. Place of birth (city/municipality, province)
6. Whether or not registered late.
   If registered late, please state the year when it was registered.
7. Complete name and address of the requesting party
8. Relationship to the child
9. Number of copies needed
10. Purpose of the certification (purpose for marriage)

No Record of Marriage /CENOMAR  (will take 5 – 10 days)

1. Complete name of the person
2. Complete name of the father
3. Complete maiden name of the mother
4. Date of birth
5. Place of birth
6. Complete name and address of the requesting party
7. Number or copies needed
8. Purpose for the certification (purpose for marriage)

The processing period for requests for the Certificate of No Marriage Record is longer than that for copy issuance requests. The period could be longer if the surname of the subject person is common such as Aquino, Ferrer, Cruz, De Guzman, and others.
